Unable to claim reward card

Hi everyone, I have had multiple emails to remind me to claim my reward card but I am unable to do so. I just get a header and footer showing when the page loads. I have tried on numerous occasions and using multiple browsers but have had no luck. Is there any solution anybody happens to know of? Thanks in advance.
