I have just received and installed a new Router to replace a faulty one. I cannot now access My network on the phone app to set up my discs and hybrid connect, What do I need to do?
Do I need to reinstall the app on my phone as it is not seeing my home network.

Access the new new hub from an ethernet connection and change the Wi-Fi SSID and password to that of the old router otherwise you'll have to seek the new Wi-Fi SSID and enter a new password on all of your wireless devices
.......and turn off Smart Setup while you're at it.
The easiest way is to change the SSID of your new hub to the same as that of your previous hub also change wireless password to same as previous hub and devices should connect like it was previous hub
