How do we cancel our mothers contract, she had dementia and is going to a care home

How do we cancel the BT contract for our mother who is suffering with Dementia? she has suffered an rapid onset and has been in hospital for over 4 months, we were told she now needs to go into care.

Yes we have both finance and health and welfare poa
