Refer a Friend Amazon Voucher

I referred one of my friends, who signed up for the BT broadband and TV package. It was confirmed by an email. 

On 23rd February, I received an email after his broadband was activated saying "we’ll email you both a £50 Amazon  Gift Card on us within 30 days". However, it has been almost 4 months since then and neither of us received the vouchers.  Any advice on how to claim that? Thanks in advance.
