Unable to claim reward card

Hi,

I am having some issues trying to claim my reward card following a new broadband order a month or so ago. When I follow the link, I am presented with 2 different account numbers however one is an old account and shows no reward and the other (new) account just presents a blank page. Please can I have some support?
