Caller re-direct

We removed this in November. It was not on the bill in November or Jan and has now re-appeared in January. How can we get this removed permanently and the money refunded?

As an aside, why is it not possible to remove this online and why does BT not ask people if they want to retain it every 12 months? Bit of a conflict here.
