I have received an email from BT stating that my Xbox ultimate game pass has been activated at a cost of £10 per month, I did not request this service, I have cancelled the auto renewal of the subscription and changed my security sign in for BT.
Any advice on how this has occurred.

Sorry this has been ordered without your consent first step we usually recommend is to change your security which you've said you're done already.
We can look into this and make sure no charge appears on your bill.
