The local exchange is not involved with your DV connection, so callers that are still connected to the local exchange PSTN, still have to be routed back to your connection via the network.
If they were also on DV, then there would not be an issue. In fact, once everyone is on DV, theoretically, routing issues should go away.
As it is at the moment, there are lots of routes to be sorted out, I can only imagine what the problems are likely to be.
Just to add to what I have said, BT Business customers that have been migrated to Cloud Voice, which is a different VOIP platform, are getting similar issues like missing calls, or no incoming calls. Many complain on the BT Business forum, as you can imagine the money they are losing.
Its usually small businesses that relied on analogue phone lines, as most large organisations moved to VOIP years ago, when the closure of ISDN30, which is was PSTN based, was announced.

The real problem is that people may not realise that some calls are not connecting, they could be important calls.
That's whats worrying me. I was hoping by posting on here one of the moderators would have picked this up. I have seen they have done this on other posts and solved problems for them with the similar issues.
